Title: Improving Operational Efficiency of Primary Health Care Centres through an innovative Management Capacity Strengthening Intervention

Abstract

Background
Primary Health Care (PHC) is the cornerstone of sustainable health systems for Universal health Coverage (UHC). Despite efforts to revitalize the PHC system in Nigeria, outcomes have remained suboptimal with Kaduna state not left out despite significant investments and commitments. This is attributed to weak accountability and capacity in governance and management of frontline PHC managers. TAConnect through HSDF supported Kaduna State Primary Health Care Board to implement the Kaduna State PHC Management Strengthening Project [KPMSP] based on findings from an initial PHC system diagnostic. The goal of KPMSP is to improve the management capacity of PHC Managers as a spur to improve quality and uptake of services.
Methodology
The project was implemented using a mix of systems development and capacity building of 362 PHC managers through in-class training across 7 key management domains in a phased implementation approach across all the 23 LGAs in Kaduna while a mentorship scheme was instituted for quality assurance.
Results
There was a significant reduction in unapproved absenteeism from a baseline of 44% across supported PHCs to 22%. The percentage of PHCs with institutionalized absenteeism reduction processes increased from 64% to 100% while the percentage of PHCs where analyzed data is disseminated increased from 18% to 96%. The percentage of PHCs with trained Quality Management teams in place increased from 50% to 100% and there was also an increase from 94% to 100% of PHCs with up-to- date quarterly business plans. Percentage of PHCs without loss of key equipment and infrastructure increased from 51% to 90%.
Conclusion
The PHC management capacity strengthening intervention in Kaduna State has boosted the managerial competencies of PHC facility managers and improved operational efficiency of PHC Centers.

Biography

Dr. Olayiwola Jaiyeola is a Public Heath Physician and health development expert with over 15 years’ experience in RMNCH and HIV programs in both public and private health sectors and across three major international development organizations. He started his career with fhi360 under the GHAIN and SIDHAS projects as a Community Health Specialist and later rose to Senior Technical Officer for Prevention, Care and Treatment before joining Pathfinder International as a Senior Technical Advisor for PMTCT where he served as the Technical Lead for the USAID funded PMTCT private sector grant. He later joined Jhpiego as a Project Director of Global Projects, where he provided leadership and oversight for two global awards by BMGF and Unitaid: The Expanded Family Planning Choices and HIV Self Testing Africa Projects.
