Client satisfaction regarding services provided by community clinics of Bangladesh: New era in primary health care delivery system
Abstract
Background: Bangladesh government has revitalized community clinics as part of its commitment to provide primary health care services at doorstep of rural people and its establishment is an excellent example of public-private partnership. Bangladesh is making remarkable and rapid progress in family planning indicators. Hence, this study was designed to assess the level of client satisfaction at selected community clinics.
Methods: A cross-sectional study was carried out from January to December 2022 and 3 weeks of data collection period at four community clinics under Laxmipur district of Chittagong division, Bangladesh. Data were collected with informed written consent and applying convenient sampling technique with the help of pre-tested semi-structured questionnaire by face to face interview. Level of satisfaction was measured by using Likert scale. Data were analyzed using Software Package for Social Sciences (SPSS) version 22.
Results: Among 303 respondents, 85.5% were female, 25.7% were in 30-34 age group and 25.4% were 20-24 age group. Around 93.1% were Muslim, 34% were housewife, 39.6% had higher secondary level education. Most (84.8%) of the clients had marriage between 15-20 years, 45.5% had 1-2 children, 74.6% were satisfied with location of community clinics, 48.5% were satisfied with waiting time. However, 38.9% were very dissatisfied with waiting room facilities, 48.5% were dissatisfied with sanitation facilities, 52.5% with safe drinking water provision. Out of pocket expenditure had statistically significant association with location of CCs. Overall satisfaction level had statistically significant relationship with sex of respondents, family type, state of housing, living with senior citizen, safe drinking water provision, fear of infertility, gender preference for baby, fear of depot injection cause irregular menstrual bleeding (p<0.05).
Conclusion: Location, waiting time, service providers’ availability, performance and knowledge, adequacy and quality of family planning logistics were satisfactory but safe drinking water provision, sanitation, waiting room facilities were the major areas of clients’ dissatisfaction at the CCs. Strict supervision and administrative scrutiny, adoption and improvement of facility infrastructure is needed for client satisfaction.

Homaira Ahmed has completed her MBBS from Delta Medical College, Dhaka and Masters’ of Public Health (MPH) in Hospital Administration from Bangladesh Medical University in 2023.She is working as Physician at GCC Medical Center, Dhaka, Bangladesh.
