Patient-care: A machine learning-based real time decision support system for public health, integrating epidemiological modeling and blockchain technology
Abstract
Public health infrastructures worldwide face increasing pressure from the emergence and re-emergence of communicable and non-communicable diseases, including dengue, malaria, COVID-19, and cancer. Timely detection of susceptible population segments and dynamic monitoring of disease spread are critical for effective intervention. In response to these challenges, we propose PATIENT-CARE, an integrated real-time Decision Support System (DSS) that leverages epidemiological modelling, machine learning (ML), blockchain technology, and data anonymization to enhance public health surveillance and decision-making. The system incorporates a feedback-based extension of the traditional SIR (Susceptible-Infectious-Recovered) epidemiological model, improving prediction accuracy by accounting for reinfection dynamics and socio-demographic factors. We combine this model with ML techniques — including auto-encoders and time-series clustering — to forecast disease progression and quantify socio-demographic impacts. To ensure data privacy and trustworthiness, the platform uses a consortium blockchain for decentralized storage and secure access control, alongside advanced anonymization mechanisms that protect sensitive patient information while supporting analytical utility. Our evaluation demonstrates that PATIENT-CARE outperforms traditional SIR models in capturing real-world disease trends and delivers superior predictive performance compared with state-of-the-art machine learning baselines. By integrating epidemiology, artificial intelligence, and decentralized data governance, this system offers a scalable and secure framework for real-time public health decision support, vital for policy makers, healthcare administrators, and frontline responders.
