Session Overview

This session examines how primary and community care can support healthy ageing, functional independence and quality of life while responding to the increasing complexity of health needs in older populations. It moves beyond disease-specific geriatric management toward prevention of functional decline, early identification of frailty, multimorbidity management and coordinated long-term care.

The session provides a platform for research on healthy ageing, frailty, falls, multimorbidity, polypharmacy, cognitive health, functional ability, ageing-in-place and community-based care. A key focus is how primary care can identify risks early and coordinate interventions that help older adults remain independent within their homes and communities.

Particular emphasis is placed on the shift from disease-centred geriatric care toward function-centred and person-centred primary care.
