Biomechanical study about the radioulnar impingement after the Sauve Kapandji procedure

Abstract

We conducted a cadaveric biomechanical study in order to determine the radioulnar impingement after the Sauvé Kapandji technique. This is the most performed procedure nowadays for treating the pain in the radioulnar articulation after instability or arthrosis, postraumatic or degenerative. We studied the distance between the distal radius and the ulnar stump in supination, pronation and neutral position with loadings of 2 up to 10 kg and without.We performed the Lluch technique as it is the most stable and with the best results in the literature, consisting in a very distal osteotomy leaving an ulnar gap of 0.5 cm. We found stability in the gap, an increase in the impingement in supination but a decrease in pronation with the increasing loading, as well a decrease of it in neutral position. Our system is unique and allow us to check the axial loading in the forearm as well as we have our own mesurement system. We concluded that the secondary stabilizers are mandatory to try to preserve during this surgery.

Biography

Marta Martínez Calvo works at the Regional Hospital of Inca in Mallorca, where she is part of the Spine Unit. She has been living on the island for the past four years, but before that, she spent six years in Pamplona specializing in hand surgery. Her passion for upper limb surgery began during her orthopedic residency in Castellón and was further strengthened during her fellowship in Bern under the mentorship of Dr. Fernández. She was born in Madrid and studied Medicine at the Autonomous University of Madrid before completing her residency training in Orthopedic and Trauma Surgery at the University Hospital of Castellón. During her residency, she conducted research on the Sauvé–Kapandji procedure under the supervision of her thesis advisor. Although she currently practices as a spine surgeon, she continues to support her colleagues in the Hand Surgery Unit with their most complex cases and enjoys sharing the knowledge and experience she gained throughout her years dedicated to upper limb surgery.
